The Midview Local Schools will continue its partnership with Final Forms for the 2021-2022 school year for all registration and athletic documentation purposes. The district is excited about the ease in use its forms and technology bring to the Midview community!
All Athletic form submissions will remain online and must be completed prior to any official practice or tryout. A student’s 2020-21 information was transferred to the new school year. A parent or guardian will simply have to login and verify the information is accurate before digitally submitting his/her signature. Students will also have to login and digitally sign various forms. Please check your spam folder if you did not receive emails from "mailman@finalforms.com".
Final Forms allows families to complete their portion of the OHSAA Pre-participation Physical Evaluation digitally. They can then print the half-completed form and take to the doctor’s office, who will complete his/her portion and make the physical valid for 13 months. Completed physicals can be uploaded through the system—please make it a clean PDF scan and not a phone photo— but the preferred method remains that a hard copy is turned into the Athletic Office.
Final Forms has a robust support network that can help you with any questions you may have during registration or while using the system. Please try that first.
