Midview Local School District partners with FinalForms, an online forms and data management service. Through FinalForms, parents/guardians can conveniently complete and electronically sign enrollment, back-to-school, and sports participation forms for their students. FinalForms saves data from year to year and season to season, meaning there is no need to enter the same information twice. FinalForms also pre-populates information wherever possible for each student, streamlining the process and saving time.

Whether you have a new phone number, switched healthcare providers, or wish to include an additional email address, parents/guardians may review their data at any time and make updates as changes occur.

All forms must be reviewed, updated as necessary, and signed annually at the beginning of each new school year. This quick and easy process will take families less than 10 minutes!
